Financial Disclosure and Payment Policies
Thank you for choosing Milestone Physical Therapy & Training for your care. To help ensure a smooth experience, please review the following information regarding our financial policies:
Co-pays & Deductibles
- Co-pays and deductible payments are due at the time of your appointment.
- For high deductible plans, we request a payment of $60 per session to be applied toward your deductible.
- Once your insurance claim is processed, any remaining balance will be billed directly to you.
Cancellation Policy
- If you need to cancel or reschedule your appointment, please let us know at least 24 hours in advance to avoid a cancellation fee.
- Cancellations made with less than 24 hours' notice will incur a $25 fee.
- To cancel or reschedule, please call us at 317-207-7536.
Card on File Policy
- We kindly ask that you provide a credit card to be kept on file. This card will be used for:
- Co-pays and payments toward your deductible at the time of service.
- Any outstanding balances after insurance has paid their portion.
- Once your insurance has processed the claim, we will notify you if your balance exceeds $250.
- If the balance is not paid within 5 days of notice, the amount will be charged to your card.
- A receipt for any charges will be provided.
